Receipt Requirements
Ensuring your receipt format is correct will help our team process your claim quickly and reimburse your treatment costs. Here is some handy guidance you use to can check if your receipt will be accepted before sending off your claim:

- None of your details
- No treatment details
- No practitioner details
- No paid status
If your first receipt doesn’t meet our requirements, don’t worry! Just get in touch with your practitioner and ask them for an invoice on letterheaded paper.

Accepted receipts can be in the form of an official invoice or a till receipt. It doesn’t matter which one you provide, as long as it features all of the following information:
- Headed paper
- Your name
- Practice name
- Practice address
- Contact details
- Date of receipt
- Practitioner’s name
- Treatment description
- Total cost
- Paid status
